All student-athletes are required to have a form uploaded to their registration account signed by a practicing physician certifying the athlete is cleared to participate in high school athletics. Physical examinations are valid for one calendar year from the date of the exam.
The athletic office does not accept any paper copies.  Please log in to your athlete's account to upload the form. Only upload page 4 into the sytem. To update a physical in the athlete's file, log in to the account and upload the new physical form in your family account.
